Winner(s) must personally claim his/her prize at the District Governor's office and present two valid identification documents (e.g. driver's license, passport, PRC license etc.) together with the winning ticket. If the prize is not claimed within sixty days of notice to the Rotary Club President, which shall be forfeited in favor of Rotary District 3810.
